POSITION SUMMARY

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute and chronic health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.

Understanding Clinical Impacts * Providing Solutions to Constituent Needs * Providing education to promote health engagement * Working closely with members to implement an individual plan of care

BACKGROUND/EXPERIENCE desired:

5 years clinical practice experience

Bilingual preferred

Disability case management experience preferred

3-5 years clinical practice experience preferred

Case management experience preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse (RN) is desired

Nursing/Certified Case Manager (CCM) is desired
