Oncology Center Social Worker/Navigator, Full Time

Performs initial and ongoing assignments of oncology patients in the Cancer Center, evaluating each patient’s ancillary needs (i.e., psychosocial, pain management, nutrition and support) and assists in arranging for each patient to have those needs met. Available to assess oncology patients for Radiation Oncology, The Breast Center, and each of the oncologists’ offices in the Cancer Center. Provides information and referral services to individuals contacting the Cancer Center. Works closely with all appropriate ancillary departments as well as the community and external organization (i.e., American Cancer Society, National Cancer Institute, etc.) to provide services and educational programs for cancer patients and their families. Provides social work assistance to patients, families, and significant others to enable them to deal with the psychosocial issues which impact illness and to maximize benefits from health care services. This position includes psychosocial assessment, treatment planning as a member of multi-disciplinary treatment team, referrals to appropriate community resources, and networking with community services providers.
