Patient Care Navigator – Joint Care Center

The Patient Care Navigator guides patients through their hospital stay by providing assistance with access issues, developing relationships, with service providers, tracking interventions and outcomes. Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assist with accessing clinical and support services. The Joint Care Center is a 21 bed orthopedic unit focused on post surgical recovery.

Some responsibilities:

  • Follow care for all Joint Care Center patients from pre-admission to discharge

  • Act as touch point for the Coastal Orthopedic Group and it's patients having surgery

  • Act as resource for patients and physicians

    Qualifications

    RN with a Bachelors’ in Nursing strongly preferred.

    2 years Ortho experience

    State license as an RN, BLS and ACLS
