Kaiser Permanente Pain Management Coordinator/Navigator RN in Halethorpe, Maryland

The Pain Management Coordinator/Navigator will assist in developing and coordinating the care of the pain management patients within the Pain Management Program. The individual accomplishes these outcomes collaboration with the clinicians of the Pain Clinic and provision of services such as multidisciplinary pain management groups and collaborative visits with the pain management clinic providers.

Essential Responsibilities: - Collaborates with Pain Management Clinicians in the assessment, management and treatment for patients with complex biopsychosocial needs. Co-leads the series of multidisciplinary group visits. Develops mutually agreed upon treatment plans with patient/family in collaboration with pain management clinic providers and primary care providers to ensure smooth functioning, high quality, affordable, coordinated care for members with pain and their significant others. Selects and obtains other services for patients with chronic pain as indicated by patient need. - Under the direction of the Pain Management leadership team and Pain Management physicians, coaches clinic staff to ensure smooth operation of pain management clinic/program and includes the following: pain-self management groups, medication management, patient intervention and medical procedures. Collaborates with Medical Lead/Chief and Patient Care Manager/Lead Nurse to develop a highly functional multidisciplinary team. Collaborates with Patient Care Manager and Medical Lead in the development of processes, procedures and systems to assure efficient delivery of high quality, affordable, integrated pain management. Provides clinical nursing support to Patient Care Manager. - Regional consultation and education: Provides consultation, education to physicians, nurses, pharmacists, other staff re: clinical care of patients with chronic pain. Works with regional and national Kaiser Permanente pain management champions on the development of practice resources and best practices that impact the care of patients with chronic pain. Is a resources to KPHC in the development of tools to improve chronic pain management. - Collaborates with all Pain Management Department/Team to evaluate and improve pain management services.

Qualifications: Basic Qualifications: Experience - Minimum three (3) years of experience in nursing - Minimumone (1) year of experience in behavioral health. - Minimumone (1) year of experience treating patient with chronic pain. Education - Bachelor's degree in nursing required, License, Certification, Registration - Current licensure as a Registered Nurse in the State of Maryland (or compact state) or able to obtain prior to date of hire. - Current Basic Life Support (BLS) certification from the American Heart Association must be obtained prior to hire date.

Additional Requirements: - Working knowledge of state health care laws, licensing board regulations and controlled substance regulations. - Working knowledge of pain management. - Comprehensive knowledge of chronic disease management. - Basic knowledge of statistics, data analysis and research design and methodology. - Knowledge of adult learning theory. - Development/implementation of quality and utilization monitors. - Policy, procedure, and protocol development. - Ability to develop treatment plans with patient/family in collaboration with pain management clinic and primary care providers. - Ability to provide consultation, education to physicians, nurses, pharmacists, other staff regarding clinical care of patients with chronic pain. - Proficiency in use of Microsoft Office software and mastery of word and excel programs. - Excellent written and verbal communication skills; public speaking and presentation skills; strong prioritization and organization skills. - Demonstrates customer-focused service skills. - Ability to coordinate group sessions.

Preferred Qualifications: - Three (3) years of experience in behvioral health. - Three (3) years of experience treating patient with chronic pain. - Work experience at Kaiser Permanente. - Work experience in supervision of professional and non-professional staff. - Masters prepared. - Working knowledge of Kaiser Permanente. - Collaborative practice models. - Pain Management.
