Licensed Referral Case Manager - Adolescent Residential Treatment Program

The Behavioral Health Center provides four separate inpatient program to ensure that people receive individual care in settings that are appropriate for their needs. Typically the treatment structure involves group therapy, individual counseling, educational groups, recreation therapy activities, and support services.

Throughout evaluation and treatment, we focus on how the patient will succeed after leaving us. Our goal is to help each person successfully re-enter his or her normal life in a timely manner and with appropriate follow-up care.

  • Special Care Adult Program (for adults 18 years and older)

  • Daybreak Adult Program (for adults 18 years and older)

  • Teton Peaks Adolescent Residential Treatment (for children ages 12-18)

  • Adolescent Acute Care Program (for children ages 13-17)

As an integral member of the Treatment Team, the BHC Teton Peaks Licensed Case Manageris responsible for the referral coordination and discharge planning/care coordination of Teton Peaks Residents.

Intake Duties:

Coordinates admissions to the program.

Coordinates with the Admission and Referral department for the BHC to ensure the referral process is completed in a timely manner, allowing for admission process to flow seamlessly and quickly.

Reviews referrals, collects additional information as needed, and presents referrals to the psychiatrist for decisions regarding admission to the program.

Discharge Planning & Care Coordination:

Will be responsible for the coordination of each resident's discharge planning.

The Case Manager will maintain contact with referring sources including individuals or community and State agencies.

Each resident's progress will be reported to the referring agency per that agency's needs or contract with Teton Peaks.

This will be done in conjunction and cooperation with the Teton Peaks Treatment Team.

Such planning will be done in cooperation with the Treatment Team.

The Case Manager may notify referring agencies of incidents warranting occurrence reports.

Fills in for Utilization Reviewer as needed. Adheres to hospital policies & procedures & demonstrates compliance with HCA Code of Ethics.

*Must be 21 years or older

*Licensed Master's Therapist in the State of Idaho possessing at least two years of relevant clinical experience.

*Position has frequent interaction with physicians, nursing staff, administrative personnel; therefore, must have the sensitivity, maturity and ability to communicate clearly and concisely, both verbally and in writing. Must have knowledge of Hospital and department policies and procedures. Must possess general knowledge of the principles and techniques of residential health care as well as an understanding of residential programming. Must be capable of establishing and maintaining a cooperative working relationship with other staff members.

*Maintain current CPR certification.

*Maintain current NVCI certification.

*Maintain current Idaho drivers license.(Employees who are not permanent residents of Idaho must have a valid driver’s license from the state in which they permanently reside.)
