Responsible for delivering care, including patient education and advocacy, to assigned patients in compliance with regulatory agencies and hospital and department policies and procedures.

  1. Performs assessment of life time risk for breast cancer on patients with strong family histories of breast cancer, as directed by the Radiologist.

    1. Uses Sarah Cannon to document all Cancer Patients and uses Omnicare navigator tracking system to document outcomes and status of patients in the system noting subsequent exams, therapies and surgery.

    2. Provides reports as needed to evaluate the program.

    3. Attends oncology and related medical staff meetings.

    4. Assists the business growth team in educating area physicians on this service.

    5. Participates in community events related to women’s health, educating public on the program.

    6. Serves as a resource for all cancer patients by performing the following duties: Contacts referring physicians regarding the findings and radiologist’s recommendations. Obtains orders for additional testing. Assists with scheduling appointments, and obtains insurance information and physician’s orders. Educates on subsequent procedures, exams and therapies that may be ordered. Provides emotional support and survivorship planning and Distress Screening for the patient and caregiver. Remains available to accompany the patient to surgery or MRI. Communicates any concerns, changes, or social needs in patient health to the appropriate provider and caregiver. Initiates referrals to hospital and community resources to connect patients to resources and support systems.

_REQUIRED EDUCATION:_ Graduate from an Accredited Nursing program

_REQUIRED LICENSE/CERTIFICATION:_ Current Florida RN License and American Heart Association BLS.

_REQUIRED EXPERIENCE:_ One year RN experience

_PREFERRED LICENSE/CERTIFICATION:_ Oncology Certification in 18 month

_PREFERRED EXPERIENCE:_ 3 years RN experience _ _
