Job Description

Participates in community outreach and enrollment activities. Provides assistance/ guidance to members, family members and caregivers. Serves on an inter-disciplinary team that provides care coordination, managing the medical, social, and behavioral health needs of enrolled members.

Responsibilities Include:

  • Participates in community outreach and enrollment activities.

  • Performs outreach to program candidates among medically underserved populations within the community.

  • Interviews candidates, family members and caregivers to compile and document relevant information (demographic, economic, social, institutional, etc).

  • Completes related forms (consent, FACT-GP, Health Risk Assessment). Performs administrative assessment of candidates’ eligibility for social services and care management programs.

  • Obtains required signatures and maintains required documentation in accordance with program policies and procedures and State/Federal regulations.

  • Describes available services and advantages of various care management activities. Provides program information and materials to candidates.

  • Participates in developing relationships with referral sources to facilitate enrollment.

  • Provides sensitive, culturally diverse assistance/guidance to patients, family members and caregivers.

  • Serves as a point-of-contact to program members, family members and caregivers. Encourages use of program resources to maximize wellness and independence

  • Assists members in scheduling appointments for consultations, diagnostic testing, program meetings, etc.

  • May travel to member’s home, medical appointments, and/or meetings to provide non-clinical support, as directed.

  • Collaborates with clinical and non-clinical team members to maintain proper workflows, procedures and documentation.

  • Participates in team meetings to provide non-clinical input regarding member status and activities.

  • Maintains integrity of related records in accordance with system policies and procedures and State and Federal regulations.

  • Develops and generates reports summarizing program activities, as required by management.

  • Performs related duties, as required.

Qualifications

  • Bachelor’s Degree in Child & Family Studies, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Physical Therapy, Psychology, Recreation, Recreational Therapy, Rehabilitation, Social Work, Sociology, or Speech & Hearing, required OR Bachelor’s Degree in any field AND five (5) years experience working directly with persons with behavioral health diagnoses, required OR Credentialed Alcoholism and Substance Abuse Counselor (CASAC), required.

  • Minimum of two (2) years experience providing direct services to persons with mental illness, developmental disabilities, alcohol or substance abuse OR linking persons with mental illness, developmental disabilities, alcohol or substance abuse to essential services, required.

  • Valid NYS Driver’s license, required.

  • Ability to communicate and interact effectively with patients and families of diverse cultural backgrounds.

  • Must be able to travel to Queens, Nassau and Suffolk Counties
