Under indirect, or direct supervision & based on the understanding of patient variables such as age, sex, & other physical characteristics: monitors the medication therapy of hospitalized or ambulatory patients as assigned; confers w/ & assists medical personnel in the care & treatment of these patients; provides medication information & pharmacokinetic dosing advice; adjust doses, orders laboratory tests, & administers medications according to established policies & procedures; monitors medication distribution; dispenses, compounds, & distributes pharmacy products. Perform other duties as required.

Essential Functions: - Upholds KP's Policies & Procedures, Principles of Responsibilities & applicable state, federal & local laws - Monitors the medication therapy of specific hospitalized or ambulatory patients as assigned to assure optimum therapeutic results & cost-effective prescribing - Reviews laboratory test results & the patient's progress to detect adverse drug reactions, & confers w/ & assists medical personnel in laboratory test interpretation & treatment of patients - Interviews patients to obtain information regarding medication use, medication allergies & sensitivities - Documents the information in the patient's medical record or Pharmacy Information Management System (PIMS) when appropriate - Advises patients of directions for use, medication storage requirements, importance of compliance, precautions & warnings for medication therapy - Advises the patient on the use of related devices & the coordination of drug therapy w/ diet, according to established polices & procedures - Provides medical personnel w/ medication information & pharmacokinetic dosing advice - Orders & adjust dosages, orders laboratory tests, & administers medication under protocol according to established policies & procedures - Monitors the effectiveness & accuracy of institutional medication distribution - Reviews physician orders & verifies accuracy of transcription data entry into medication administration record or Pharmacy Information Management System (PIMS) - Verifies accuracy of medication preparation & dispensing by pharmaceutical personnel - Reviews patient's medical record to verify proper delivery & correct administration of medication - Reports discrepancies to appropriate medical and/or administrative personnel - Develops & tests requirements & specifications for protocols, computer programs, pharmacokinetic dosing & other pharmaceutical care applications - Participates in assigned education & training programs - Develops & maintains teaching curriculum & conducts educational programs for pharmacy, other medical personnel, & patients - Participates in rounds, hospital committees, utilization review & quality management programs - Responds to medical emergencies according to established procedures - Assume other activities & responsibilities from time to time as directed

Qualifications: Basic Qualifications: - Doctor of pharmacy (Pharm. D), preferred - Licensed by the California state board of pharmacy - American Heart Association (AHA) approved Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification must be obtained within the probationary period - National Provider Identifier (NPI) required - Possess comprehensive knowledge of current pharmacological/ biopharmaceutical principles, medical terminology, pathologies & disease states, & the ability to relate this information to the patient's medical management plan - Comprehensive knowledge of emergency medications, & the ability to become CPR-certified - Working knowledge of aseptic technique & the ability to prepare sterile IV products & chemotherapeutic agents - Excellent verbal & written communication skills - Knowledge of federal & state laws & regulations regarding the provision of pharmaceutical services - Ability to extract pertinent information from the patient, or other sources, & relate it to the patient's medical management plan - Ability to learn & operate pharmacy computer system(s) & other equipment - Ability to type medication labels - Ability to teach & evaluate the effectiveness of teaching
