We are a full-service, acute-care hospital with 317 inpatient beds located in the heart of Orange Park, providing medical care to Clay County and surrounding communities. The hospital has 170 private rooms. There are nine labor, delivery and recovery room suites for new mothers and a seven-bed, Level-2 neonatal intensive care unit (NICU) for premature babies and those born with special needs. There is also a 24-bed psychiatric unit, 20-bed inpatient rehabilitation unit and 20 intensive care unit(ICU) beds.

_Position Summary:_

The role of theSocial Workeris to integrate as a team member with Case Management in the discharge planning process. The Social Worker contributes to patient care through an understanding of social elements in relation to hospitalization, disability, illness, financial support, or other issues. The Social Worker provides assistance to patients and their families to achieve optimal outcomes from their medical care. The Social Worker works collaboratively with Case Managers on utilization review processes as needed, and assists with other case management responsibilities as directed. _ _ _Essential Duties:_ * TheSocial Workercollects data about patients and identifies individual needs to develop a 41c98omprehensive cost-effective discharge plan. This involves identifying biopsychosocial and post-hospitalization needs of patients of all ages, making necessary arrangements to assure continuity of care. * TheSocial Worker, as early in hospitalization as possible, identifies patients who are at risk for discharge planning barriers and/or who have potential for extended lengths of stay. * TheSocial Workerassures ongoing communication through documentation and re-assessment the patient and the discharge plan, considering the anticipated needs at the present and future levels of care. * TheSocial Workerreceives referrals from case managers, physicians, patient/family, or any member of the multidisciplinary team. The case management segment of the Nursing Admission Assessment is one method by which patients who may need assistance after discharge are identified and referred. * TheSocial Workerprovides education about community resource for patients and families to support realistic choices about post-hospital care plans and to facilitate the implementation of the plan in conjunction, as appropriate, with the payor source. * TheSocial Workermaintains extensive and current knowledge of community resources, discharge planning and resource to nursing homes, extended care, rehabilitation facilities, homeless shelters, and dialysis clinics, in the local area and at distances. The Social Worker serves as a primary source of information about community resources, and maintains working relationships with numerous external agencies.

_Education:_ * A Master's Degree in Social Work is required _License/Certification:_ * A current American Heart Association Basic Life Support (BLS) Provider is required * A Master's Level License is preferred - LCSW, LMSW, LPC, LMHC _Experience:_ * At least 3 years previous experience as a Social Worker is required for this position * Previous Hospital experience is preferred * Previous Trauma experience is preferred _Shift:_ * Typically Monday through Friday position * Rotating weekends and rotating holidays are required
