POSITION SUMMARY

Working with dual eligible members as a care manager to coordinate benefits and care, educate on disease processes and assist the member to be able to self-manage their behavioral and physical health care needs.

Fundamental Components:

Must have behavioral health clinic or BH care management experience, understand physical aspects of chronic care conditions and be able to guide members to a healthier lifestyle while decreasing utilization of ER and inpatient stays.

BACKGROUND/EXPERIENCE desired:

Must have a degree (BS, MS) but does not need to be licensed.

2 years experience in behavioral health, social services or appropriate related field equivalent to program focus is required

Managed Care experience preferred

Case management and discharge planning experience preferred

EDUCATION

The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
