POSITION SUMMARY

CM will be working with dual eligible members who may have physical and behavioral health issues. Care coordinating with providers to ensure members benefits and needs are addressed.

CM will be working with dual eligible members who may have physical and behavioral health issues. Care coordinating with providers to ensure members benefits and needs are addressed.

BACKGROUND/EXPERIENCE desired:

3-5 years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.

Case management and discharge planning experience preferred

Managed Care experience preferred

Behavioral Health/Crisis intervention skills heavily preferred

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse is desired

Mental Health/Licensed Professional Counselor is desired

Mental Health/Licensed Clinical Social Worker is desired

FUNCTIONAL EXPERIENCES

Functional - Nursing/Medical-Surgical Care/4-6 Years

Functional - Medical Management/Medical Management - Case Management/1-3 Years

TECHNOLOGY EXPERIENCES
