Aetna Clinical Care Manager in Phoenix, Arizona

POSITION SUMMARY

Intensive Care Manager position supporting members and their assigned community-based clinical teams in addressing member needs through an integrated model of care. Position will be location in a provider clinic site(s) in the greater Phoenix metropolitan area.

Fundamental Components:

MINIMUM Bachelor's degree AND independent clinical license (RN, LCSW, LPC)

75% of time spent in clinic setting working with clinical teams and members

Use system data tools to evaluate member physical, behavioral, and social needs

Support clinical teams actions to address identified needs with appropriate use of available resources

Act as support to clinical teams and members by providing health education which supports positive lifestyle changes

Maintain member records in accordance with business guidelines in a electronic record system

BACKGROUND/EXPERIENCE desired:

3-5 years clinical practice experience (hospital setting or community based setting) required

Case/care management and discharge planning experience preferred

Experience with managed care benefits (Medicare / Medicaid) preferred

Collaboration with multiple stakeholders to coordinate care preferred

EDUCATION

The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Candidate MUST have one of the following:

Nursing/Registered Nurse (RN)

Mental Health/Licensed Clinical Social Worker

Mental Health/Licensed Professional Counselor

Certified Case Manager (CCM) along with the above certifications is a plus
