Utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal nutritional status.

Fundamental Components:

Through the use of clinical tools and information/data review, the successful candidate will use knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member's nutritional status and risks. Will utilize assessment techniques to determine members level of health literacy, technology capabilities and readiness to change. Will assess health/nutritional essentials to establish a plan of care based on members health status/risks, education, and support needs. Assist members in weight management, meal planning and healthy eating. Demonstrated understanding of the management of chronic health conditions, especially as it relates to diabetes, obesity and hyperlipidemia. Collaborate with other health professionals on the members behalf. Strong desire to coach, motivate behavior change through motivational interviewing and active listening skills


3-5 years of coaching experience Computer skills include navigating in multiple systems and web-based applications, use of mouse and strong keyboarding skills. Demonstrated ability to work independently as well as willing to be flexible working with a team. Experience with creating workflows and policies a plus. Certified Dietitian/Nutritionist is required.


The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

This is a telephonic position and may require sitting for extended periods of time. This position is full time Monday-Friday requiring the flexibility to speak with members based on the members availability.
