Kaiser Permanente Palliative Care RN Coordinator - Palliative/Hospice Care Services, Holy Cross Hospital in Silver Spring, Maryland

In general, the Palliative Care Nurse Coordinator is responsible for screening palliative care referrals/ evaluating the appropriateness of palliative care referrals, gathering patient and family data in preparation for the palliative care consult by the palliative care team, coordinating palliative care consults, assessing member palliative are needs and monitoring the quality of outcomes.

Essential Functions: - Maintain the integrity of palliative care referrals: Screen, assess&prioritize palliative care consult referrals; Schedule&plan palliative care consults for the palliative care team while being sensitive to time constraints&resource. - Judiciously conserve Health Plan resources. Clarify eligibility status in accordance w/ Health Plan policies&procedures.; Ensure the appropriate administration of benefits by accurate interpretation&by applying the benefit w/in limitations,&in a manner that exhibits judgment&a realistic understanding of issues. Identify issues, trends,&barriers that delay the patient's timely transition to the next level of care,&accurately communicate this information to others regardless of their status or position. Influence actions&opinions in cases where a physician need assistance to expeditiously transition a patient to the most appropriate level of care. Maturely express opinions&use negotiation skills to influence financial outcomes related to hospital days, alternative care&other referred services. - Provide palliative care consult services. Utilize the nursing process approach to prospectively&concurrently formulate&execute plans for health care services in collaboration w/ MAPMG&community physicians. Identify where - support structures exist, identify appropriate referrals&resources&take action&make commitments to solve individual patient care mgmt problems. Develop creative patient care alternatives to ensure the delivery of high quality, cost effective care using analytical problem solving as well as intuition to stimulate new waves of thinking. Discuss complex or difficult individual cases w/ supervisor, MAPMG physicians&QRM physicians. Participate in discharge planning. - Provide quality screening. Demonstrate competency to assess all aspects of care across the continuum for variations from quality&utilization standards. Seek clarification of incomplete or illegible records including inpatient, outpatient&contracting physician office records&report problems for corrective action. Identify&report potential risk cases or situations by presenting clear information that builds credibility. Exhibit a commitment to the continual improvement in the quality of patient care services. - Demonstrate competency to respect patient rights. Display an awareness of individual rights of patients&educate the member about those rights. Maintain confidentiality of patient records, information,&departmental activities involving patient information. Practice w/in ethical&legal guidelines using established policies&procedures for guidance. - Coordinate communication&promote collaboration between the disciplines providing care or case mgmt w/in&outside the Kaiser Permanente system. Optimize productive working relationships by functioning as a liaison, problem solver,&consultant for community social services, health care agencies, providers&various Kaiser Permanente departments. Provide case mgmt, referral mgmt,&benefit interpretation for attending physicians, service chiefs, area physician mgrs,&medical facility mgrs. Work w/ Medical Center staff&physicians to develop appropriate plans of care for individual patients or groups of patients in such a way as to build high morale&group commitment to goals&objectives. - Demonstrate competency to perform as a team member. Collaborate w/ other QRM staff by assisting w/ routine department functions in a helpful manner. Seek mgrial guidance prospectively in cases w/ high cost potential where benefit mgmt is an alternative, when quality or utilization issues will have a financial impact on Kaiser Permanente or when issues need resolution or clarification. Train&orient assigned personnel&alert mgr to difficulties or situations that require the mgr's personal attention. Participate in departmental activities related to education, staff meetings,&guideline&policy development. Demonstrate flexibility based on the overall needs of the department. Independently schedule&set work priorities while being sensitive to time&resource constraints. Makes recommendations on pain&symptom mgmt to the palliative care team, MAPMG, community physicians&other disciplines involved in the member's care - Exhibit commitment to act in ways that demonstrate accountability. Maintain current knowledge of&seek opportunities for continued learning related to: Medical diagnosis; Care pathways; Nursing care; Ambulatory care; Utilization mgmt; Member benefits; Medicare benefits; Complete departmental documentation by writing w/ clarity&effectiveness. - Demonstrate competency to adhere to organizational policies, procedures,&rules. Maintain regular attendance&promptness at work in accordance w/ personnel and/or departmental policy. Observe all policies related to time sheets, fire&safety, employee health&dress standards. Abide by all policies regarding conduct in the workplace. Observe&be a model for regional service standards - Performs other related duties as directed.

Qualifications: Basic Qualifications: Experience - Minimum one (1) year of experience in acute hospital utilization review or equivalent required. Education - Successful completion of palliative care training required, including: pain and symptom management, nutrition and hydration, psychosocial and spiritual care, and hospice. License, Certification, Registration - RN License in MD (or compact state) and the District of Columbia - BLS from American Heart Association certification required. - Certified Hospice and Palliative Care Nurse (CHPN) as offered through Hospice and Palliative Care Nurse's Association required within 18 months of employment for nurses hired into the position after 7/1/2015. Current employees must maintain their certification.
