Position Summary:

The Registered Dietitian is responsible for promoting optimal nutritional health, food safety and access, for high risk commercial and Medicare members in a new community based care management program. The Registered Dietitian, working within a multi-disciplinary team, leads and implements dietary strategies and interventions that support holistic member centered care along the health continuum.

Additional Job Responsibilities:

  • Provides subject matter expertise, consultations, case reviews and education to clinical staff and members

  • Participates in the inter-disciplinary team and collaborates with healthcare team to align nutritional interventions with care goals

  • Evaluates the plan of care and effectiveness/outcomes of dietary interventions and modifies as needed

  • Educates members and families on healthy diet, appropriate nutrition guidelines for disease state(s), provides prevention and wellness, guidance

  • Connects members with nutritional community resources, dietary counseling services, leveraging applicable nutritional plan benefits

  • Collaborates with nutritional therapy providers, home care providers , community resources for nutritional support

  • Conducts home visits, as needed, to validate appropriate nutritional needs are being supported

  • Utilizes evidence based knowledge and protocols appropriate to patient populations and diagnoses

  • Adheres to care management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures

Background/Experience desired:

  • 3+ years experience as a Registered Dietitian required

  • Local travel required

  • Demonstrates strong nutrition knowledge/experience in working with people with chronic conditions e.g. diabetes, renal disease, complex nutritional needs

  • Demonstrates skills/knowledge needed to coordinate & support prescribed nutrition therapies

  • Build relationships & work collaboratively within an inter-disciplinary team

  • Bilingual (Spanish or Creole) preferred

Education:

  • The minimum level of education required for candidates in this position is a Bachelor's degree in Nutrition or related field

  • Masters Degree in Nutrition or related field preferred

Licenses and Certifications:

  • Licensed Dietitian/Nutritionist (LD/N) in the state of Florida, active and in good standing
