Primary responsibility is to focus on achievement of optimal patient health care outcomes while ensuring appropriate utilization of health care resources. Working closely with primary care teams, specialty care teams and medical providers, the Liaison Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self- manage.

Serves as the liaison across the internal Kaiser Permanente care continuum and between Kaiser Permanente and all externally contracted providers, facilities, and resources and provides feedback to the organization regarding the service and quality of contracted services. The Liaison Nurse collects data and provides input to leadership regarding issues or concerns related to utilization, cost, quality, and service and care delivery to patients.

This position is located at our St Peter hospital in Olympia with travel to Tacoma St. Joseph office TMD as needed. Hours are 8:00 - 4:30 working every other weekend with variable shifts.**

Qualifications: Education Minimum Education Requirements Bachelors Degree BA or BS. Preferred Education Bachelors Degree BS in Nursing. Knowledge/Skills Minimum Knowledge/Skills Requirements Intermediate Effective, independent nursing judgment and skills, and use of evidence based clinical decision making criteria. Advanced Knowledge in management of chronic disease process, nursing process and collaborative care planning. Demonstrated skill and experience in effectively collaborating with care team members. Experience Minimum Experience Requirements 3 years Recent RN medical/surgical/ambulatory clinical experience required. 2 years RN experience in ambulatory case management, care coordination or disease management. License/Certifications Minimum License/Certifications Requirements At time of hire Current Washington State RN license. Able to obtain shortly after hire Certification in case management field within two years of hire. Able to obtain shortly after hire CPR within 6 months of hire.
