*JobSummary**–*TheBirthCertificateClerkisresponsibleforcollectingthenecessarydataelementsforthecompletionofthebirthcertificateasrequiredbystatelaw.ResponsibilitiesincludeworkingwiththeparentstogatherdatathatisrequiredfortheAcknowledgmentofPaternity(AOP),whichcanbeacriticalandsometimesdifficultpartofthebirthcertificateprocess. *Supervisor***–FacilityHIMDirector DutiesIncludebutarenotlimitedto: · Completes anycertificationprogramandcontinuingeducationthatmayberequired bystatelawtoaccurately performthedutiesofthebirthcertificateclerkandworksundertheguidelinesandprocessasdefinedbythestate. · Interactswiththeparentsto collectanddocumentthebirthinformation, deliveringtheappropriateformstothemandprovidingguidanceinthecompletionoftheforms. · WorkswiththeparentstocompletetheAcknowledgmentofPaternity form,whichcanrequirepatience, diplomacyandsensitivityif thereisconflict regardingparental responsibility.

· Reviews patient medicalrecordsandotherresources,asneeded,toobtainrequiredbirthinformation. · Reviews completedbirthcertificateworksheetforcompletenessandaccuracy, ensuringthatsignaturesofthe parentsareobtainedforthebirthcertificateandanyotherstatespecific program,suchasHealthyStart,Medicaid,Social Securityforms. · Entersbirthcertificate information intoappropriate computersoftware programandtransmitsdatainatimelymannerasrequiredbystatelaw. · Meetsorexceedsproductivitystandardofprocessingtwo(2)birth registrations perhour. · Meetswithauditors, accordingtostateregulations,whovisitthefacilityonaregular basistomonitor/auditthebirthcertificateandAOPprocess, providingreportsanddatatothemasneeded. · Changes babynamesinMEDITECHasneeded, fromgenericbaby girl/babyboytogivennameandvalidates namechangeinHorizonPatientFolder(HPF)/McKessonPatient Folder(MPF). · WorkscloselywithObstetricalnursingstafftoperformdutiesandmayworkonthefloorandaway fromtheHIMdepartment. · MaintainsconfidentialitywithallPHIandHIPAArelatedinformation. · PracticesandadherestotheCodeofConductphilosophyandtheMissionandValuestatement. · Otherdutiesasassigned.
