St. David's South Austin Medical Center is an acute care facility with 316 beds, offering a range of complex specialties and sub-specialties, including a nationally accredited oncology program with the area’s only adult bone marrow transplant program; a trauma program that includes all of the capabilities and programmatic elements of a Level II trauma center to treat the most severely injured and critical patients; a comprehensive cardiac program; full-service maternity and newborn care with Level I and II nurseries; and two full-service emergency centers in the communities of Bee Cave and Bastrop.
POSITION SUMMARY The ED Social Worker will assist in meeting the psychosocial needs of the emergency room patients, by providing emotional support, psychosocial assessments, counseling, crisis intervention and discharge planning services to patients/families.
Respond to and collaborate with all interdisciplinary team referrals within 20 minutes while on duty. When on call, responds to page within 15 minutes and arrive at the emergency room within one hour of page if needed.
Conduct a thorough psychosocial assessment with patients who present to the ER with a psychiatric complaint. Psychiatric complaints include, but are not limited to, suicidal/homicidal ideation, mania, depression, and psychosis. Provide brief therapy and education when appropriate. Assessment includes a diagnosis and treatment plan.
Arrange for commitments and psychiatric hospitalization for patients requiring inpatient psychiatric care.
Assess and provide information, education, support, and referrals, as requested by the interdisciplinary team, for patients who present with anxiety related symptoms.
Conducts assessments of emergency room patients who present with substance abuse concerns. Provide referrals to appropriate community agencies and arrange for admission and transportation to an inpatient detox/rehab facility if requested.
Assess and make appropriate referrals for patients who present with sexual/physical assaults, or concerns about child/adult safety.-
Reports the results of all assessments immediately upon completion to the attending physician and other members of the interdisciplinary team responsible for the patient’s care, to ensure timely communication of pertinent information which can impact the patient’s medical treatment and outcome.
Documents psychosocial assessment, evaluations and recommendations prior to the end of the shift, to ensure timely communication of pertinent information which can impact a patient’s medical treatment and outcome.
Interprets and reports appropriate social and family information to the health care staff in a timely manner.
Assists patients and their families with education/clarification regarding treatment choices. Provide appropriate counseling and support to patients and their families.
Provides discharge-planning assessments to emergency room patients and their families. Arrange discharge services and transfer to an acute facility or nursing home if appropriate.
Documents in the patient’s medical record on the same day social service intervention is provided.
If a call from the community is received, make appropriate referrals as needed.
Maintains effective, positive working relationships with hospital health care staff and community professionals.
Assists with plans for High Alert Patients.
Maintains state social work licensure and post in accordance with JCAHO and NASW guidelines.
Attends and participates in staff meeting.
Demonstrate understanding of HCA’s and “Patients First” safety initiative by strict compliance to all safety protocols and procedures.
EDUCATION REQUIRED: Master’s of Social Work
EXPERIENCE REQUIRED: Two years previous medical social work experience preferred.
LICENSED/CERTIFICATION REQUIRED: LMSW or LCSW license required
QUALIFICATIONS: A thorough working knowledge of social services in a direct services health care setting is needed, including crisis intervention, support counseling and discharge planning. Must possess excellent written and verbal communication skills, particularly in face-to-face personal contact with patients and their families. Must have working knowledge of community information and resources for referrals. Ability to learn computer entry and retrieval skills. Discretion in handling highly confidential information from patients, and medical records, and the ability to work with highly diverse populations.
