Coordinates with the team, patient and family to develop a goal directed program. Must maintain accurate records of services provided. Responsible for following departmental procedures, maintaining professional standards and seeking professional growth.

_Education_ Bachelor of Science or a Master's of Science Degree in Physical Therapy

*Experience* Experience as a member of an interdisciplinary team with patients with a variety of diagnoses preferred Hospital experience preferred At least one year of experience as a Physical Therapist

_License/Certification_ Current license to practice in the State of Texas Current CPR certification
