Position Summary * Under the direct supervision of the Supervisor of Nursing Unit, and according to hospital policies and procedures, the Staff Nurse assesses, plans, implements, and evaluates nursing care for assigned patients.
Job Function *
Perform complete and accurate nursing assessment on assigned patients
Completes admission assessment accurately and timely according to unit specific policies and procedures.
Accurately documents admission information and assessment in CPN.
Identifies changes in patient condition and intervenes promptly and appropriately according to unit specific policies, procedures, and standing orders. Reports changes (i.e., test results, abnormal findings, etc.) to the physician and /or SNU appropriately.
Assesses and documents patient’s educational needs in CPN, on admission and throughout hospitalization.
Plans comprehensive nursing care for assigned patients according to written hospital standards and physician orders.
Initiates an individualized nursing care plan (NICU).
Identifies and prioritizes relevant nursing diagnoses and initiates them in CPN.
Reviews and updates nursing care plan every 24 hours.
Initiates discharge planning upon admission and accurately documents in CPN.
Provides and documents skilled care to assigned patients in compliance with written hospital policies, procedures, and according to physician’s orders.
Performs and documents nursing care according to patient needs in a competent and timely manner. Performs NICU admissions and NICU discharges in a competent, timely manner and with minimal assistance. Able to manage all levels of NICU assignments with minimal assistance. Able to organize patient care and CPN documentation so that documentation in accurate and timely.
Accurately administers and documents medications in EMAR according to unit specific policies and procedures. Verifies and acknowledges all active medications at the beginning of each shift and with changes. Double checks all medications with another RN before administration.
Provides individualized nursing interventions based on identified nursing diagnoses, plan of care, and NICU policies and procedures.
Initiates appropriate teaching and/or referrals. Notifies the NICU SNU or NICU Educator if assistance is needed in meeting the patient/parents teaching needs.
Carries out appropriate interventions based on identified changes in patient condition.
Initiates Chain of Command, as necessary, to appropriately care for patients.
Assist the surgical team through case preparation and surgical assistance. This includes initiation and completion of the NICU surgical checklist.
Evaluates patient care outcomes in an appropriate and timely manner.
Evaluates plan of care to determine need for interventions in a prompt and timely manner according to unit specific policies, procedures, and standing orders.
Documents resolution of identified patient problems in CPN and communicates unresolved problems to physician and/or SNU (if indicated).
Evaluates effectiveness of patient teaching and revises teaching plan as indicated. Accurately completes CPN discharge teaching record as teaching is completed.
Participates in Quality Improvement activities
Assists mothers in breastfeeding
Assesses breastfeeding techniques and provides support or referrals to Lactation Dept. when appropriate.
Provides lactation education and referrals to Lactation Dept. when appropriate. Uses NICU scheduling book to schedule lactation appointments for NICU mothers.
Demonstrates skill with use of breast pumps
Demonstrates knowledge of breast milk collection, handling, and storage. Double checks all human milk with second RN before feeding.
Required: Graduate from an NLN accredited school of professional nursing
License & Certifications *
Required:Texas license; If Compact Licensed, must obtain Texas license within 90 days of hire
NRP within 6 months of hire
Preferred: STABLE certification within 12 months of hire
