*JVS Job Title: Health Coach - Wellness POSITION SUMMARY

The Wellness Coach utilizes a collaborative process in conjunction with the client's health and wellness staff to plan and implement wellness events and programs to promote/influence members in decisions related to achieving and maintaining optimal health status. The goal of these programs/events is to help members achieve healthy lifestyle behaviors and align these lifestyle behaviors with individual wellness goals.

The Onsite Wellness Coach must have a strong desire to educate, motivate and coach members to achieve their optimal health status.

Fundamental Components:The goal of the wellness promotion program is to help members understand and develop awareness of healthy lifestyles and align lifestyle behavior with personal wellness goals.

Conduct telephonic and online clinical coaching assessments and sessions, as well as onsite group coaching events. Provide recommendations for healthy behavior changes according to established protocols.

Assist members in weight management, tobacco cessation, stress management, nutrition, physical activity and other healthy lifestyle behavior change programs.

. Identify co-morbid conditions, hospitalizations, prescriptions, and other factors that could impact the coaching and support needs of the member outside the scope of the health coaching program.

Support clinical integration and collaboration on member care plan in conjunction with other health professionals.

BACKGROUND/EXPERIENCE:

  • 3-5 years experience with healthy lifestyle coaching for weight management, tobacco cessation, nutrition, physical activity and/or stress management.

    *Experience in facilitating positive behavioral change. Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in new and challenging situations.

  • Background in motivational interviewing, active listening skills, Trans-theoretical Model, mindfulness based, and other evidence-based coaching techniques preferred.

  • Strong clinical decision-making skills. Strong desires to coach, motivate, and support members.

  • Ability to work with a multidisciplinary team. Understanding of management of chronic health conditions, especially as it relates to Diabetes, Obesity and Hyperlipidemia.

  • Great presentation and business acumen skills.

  • Professional communication skills both written and verbal.

  • Proficient skill level with MS Office Suite Programs: Word, Excel, PowerPoint and Outlook.

  • Strong organizational and time management skills.

  • Good at making recommendations and problem solving.

  • Reliable means of transportation, clean DMV record.

  • Able to travel locally.

    EDUCATION

    The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

    Health and Fitness/Health Fitness Specialist Certification is desired

    ADDITIONAL JOB INFORMATION. Candidate must be able to accommodate a flexible schedule to adjust to client needs. Typical hours are 9-5:30 CST M-F, but hours may start earlier or later on occasion, to meet client needs. Rare overnight travel possible. Telephonic support will be necessary as needed.

  • This position has local travel throughout the state of TN.
