Kaiser Permanente Physical Medicine Team Lead - Speech Therapy (Sunnyside) in Clackamas, Oregon
The Physical Medicine Team Lead assures delivery of high quality, cost-effective care to KP Reg-l Rehabilitation & KP Sunnyside Medical Center & KP Westside Medical Center patients by acting under the direction & in partnership w/Reg-l Rehabilitation Srvs Mgmt to provide clinical leadership for the physical medicine staff.
- The Physical Medicine Lead will assure delivery of high quality, cost-effective care to Reg-l Rehabilitation Srvs patients by: In collaboration w/leadership, determining the staffing & scheduling needs for their designated area of responsibility. Acts in a cost conscious manner to recommend canceling, teaming & mix alterations as fluctuations in demand occur. Assisting & participating in the hiring & orientation of physical medicine staff. Developing & maintaining orientation checklist & competency documentation. Assisting w/pro-active coaching of staff performance, providing input to staff evaluations, & identifying & reporting problems that impact delivery of patient srvs. Acting as a clinical resource to inter/intra-Dept staff. Identifying patient/caregiver knowledge deficits associated w/health care &/or the therapeutic regimen & responds appropriately. Assessing staff educational needs w/in the Module/Dept/Srv & facilitates the delivery of appropriate in-srv experiences. Collaborating w/Reg-l Rehabilitation Srvs Mgmt by participating in appropriate policy/procedure development, representing the physical medicine staff at meetings, & by participating in physical medicine equipment purchase decisions & maintenance plans. Assuring inter-Dept coordination between the Reg-l Rehabilitation Srvs Physical Medicine group & other groups w/in the Region by acting as a liaison to other Depts w/in KP & outside agencies. Leads case conference/huddles as appropriate. Triaging Reg-l patient referrals, managing access, wait lists & referral process in partnership w/provider leadership, appointment center & Physical Medicine Mrg.- Provides skilled therapy srvs to patients consistent w/practice & licensing standards under a care plan directed by the client's physician. Exhibits professional behavior supporting Reg-l & departmental policies, procedures goals & values. Activities include performing initial & ongoing assessments of patient rehab potential, functional needs & abilities, activity tolerance, mobility, equipment needs, compliance w/physician ordered care, psychosocial status, need for community resources & rehabilitation srvs. Actively seeks opportunity for continued professional growth & knowledge. Develops & implements an individualized care plan utilizing departmental & community resources in a cost-effective manner. Skilled plan of therapy care includes therapeutic exercises/activities, modalities, adaptive equipment based on patient assessment & understanding of treatment risks & benefits relevant to the patient's medical condition, rehabilitation potential & prognosis. Coordinates the delivery of therapy care w/nursing, social work, aide & therapy to KP & non-KP providers which results in effective utilization of resources & the timely attainment of achievable rehabilitation. Supervise therapy assistants, as applicable, through on-site evaluation of the provision of care, conferencing & review of their documentation according to State Practice Act & agency policy/procedure. Basic Qualifications:Experience- Minimum two (2) years of recent experience as therapist in the ambulatory or hospital setting.Education- Degree awarded from a physical therapy program approved by the American Physical Therapy Association OR the Council on Medical Education and Hospitals of the American Medical Association in collaboration with the American Physical Therapy Association OR an American Occupational Therapy Association-approved program OR from an accredited graduate program by the American Speech and Hearing Association.License, Certification, Registration- Current Oregon or Washington PT, OT or SLP license required upon hire/transfer.- Driver's Licenses required upon hire/transfer.- Basic Life Support (BLS) for Health Care Providers required within 2 weeks of hire/transfer.- National Provider Identifier (NPI) required prior to employment start date. Additional Requirements:- Demonstrates competency in the use of therapy modalities and equipment.- Demonstrates effective interpersonal communication.- Able to prioritize patient needs to achieve attainable treatment goals.- Thorough knowledge of teaching/learning principles.- Working knowledge of interdisciplinary care coordination.- Keyboarding or typing skills.
Preferred Qualifications:- Minimum one (1) year of leadership (preceptor, supervisor, or manager) experience or commensurate experience, prefer some budget development.- Minimum five (5) years of physical therapy, occupational therapy, or speech language pathology experience.
Experience as an interdisciplinary team member.
Working knowledge of the managed care industry.- Knowledge of Medicare regulations/requirements.- Knowledgeable of the care for the injured, chronically ill, and frail elderly.- Excellent time management, problem solving and customer service skills.- Excellent written and verbal communication skills.- Able to work autonomously and manage their caseload.- Operational knowledge of computerized clinical documentation and office application systems.
