Improves health outcomes through coordination of health education programs in Chronic Disease Self-Management (i.e. self-management programs, diabetes prevention, tobacco cessation, chronic pain, etc.) and Mental Health programs throughout the Linn, Lincoln and Benton County Region. The team’s primary focus is to create better access and opportunities for under-served populations in the region. Serves as a member of the Health Education team, based in the SHS Research & Education Department. While each team member has an area of primary responsibility, team members are cross-trained to increase team flexibility and availability to respond to system health education needs.
The SHS Health Education Team provides technical leadership, advice and support for community and patient health education programming. The team responds to regional needs with information, resources, education or programs and supports the changing health needs of our communities with a focus on physical and mental health. The team works with experts in the development of class content and deploys evidence based training programs for personal engagement and self-management.
One (1) of the following required:
Bachelor’s degree in a Public Health, Health Education, Health Promotion or related area within six (6) months of hire.
Associate's degree in a related field plus two (2) years health related experience.
Four (4) years experience in Public Health, Health Education, Health Promotion, or a related area.
Experience and/or training in basic computer software programs required.
Bilingual (English/Spanish) required.
Experience and/or training in health promotion program design, implementation and evaluation preferred.
Experience and/or training in managing program budgets preferred.
*This limited duration, pilot funded position works with multiple Regional Health Education Hub (RHEHub) partners to meet the requirements of this pilot-funded program. The RHEHub seeks to improve health outcomes through coordination of health education programs in chronic disease self-management and mental health programs in the Linn, Benton, and Lincoln County Region. The primary focus of the RHEHub is to create better access and opportunities for under-served populations in the region. This position will provide support on the implementation of health education programs in both English and Spanish (i.e. Tomando Control de su Salud and the Diabetes Prevention Program: PreventT2). There will be travel across the region and the home office for this position is in Corvallis. *
