FACILITY SUMMARY: Del Sol Medical Center is a Level II Trauma, 347 bed acute care TJC accredited hospital. Within our hospital campus, we also house an Inpatient, CARF accredited Rehabilitation Hospital. We are located on the Eastside of El Paso, Texas right off of Interstate 10. We provide exceptional patient care services to the El Paso and Fort Bliss communities, as well as Southern New Mexico and West Texas Regions.

POSITION SUMMARY: To provide leadership with regard to Healthcare reform, resource management, quality and cost effective care. Responsible for planning, directing, implementing and coordinating the case management effort at Del Sol Medical Center.

ESSENTIAL FUNCTIONS: 1. Plans and manages processes to measure, assess and identify opportunities to improve the performance of the Case Management Department as well as the total organization performance improvement effort. 2. Provides administrative direction to development and evaluation of all departmental personnel. 3. Utilizes every opportunity to educate hospital staff, medial staff, Board of Trustees to their role in the Case Management process. 4. Oversees coordination of the patient care process in the acute care setting by monitoring and evaluating these processes and the services utilized to meet an individual’s health needs. 5. Understands and models the importance of communication and collaboration in the identification and utilization of available resources to promote quality, cost effective outcomes. 6. Other duties as assigned.

EDUCATION REQUIRED AND/OR PREFERRED: (list required and preferred separately) PREFERRED § Master of Science in Nursing. REQUIRED § Bachelor of Science in Nursing.

LICENSURE/CERTIFICATION: (list required and preferred separately) REQUIRED § Current Registered Nurse License in the State of Texas or compact state. § BCLS certification . EXPERIENCE REQUIRED OR PREFERRED: (list required and preferred separately) PREFERRED § 3-5 years experience in acute care setting with strong clinical background. 1-2 years experience as Clinical Nurse Specialist. § Experience in process of Case Management, discharge planning, and quality improvement.

SKILLS, KNOWLEDGE, AND ABILITIES: PREFERRED § Knowledge and skills in management principles and application. Data referral and analysis. § Demonstrates leadership ability with good oral and written communication skills. § Ability to function independently with minimal supervision. § Program develop and present effective presentations. § Bilingual in English/Spanish.
