The Case Management AVP Facility Support is responsible for successfully improving the performance of identified facilities related to key Case Management metrics, including but not limited to length of stay, observation management, and inpatient rehabilitation referrals. This position is critical in achieving sustainable desired outcomes in our facilities through model realignment, process improvement, and education. The AVP will serve as an experienced subject matter expert in HCA Case Management functions, be able to interface and engage with both division and facility leadership, and articulate Case Management standards. This position will be directly accountable for the desired outcome of Case Management engagements, including tools, training, tracking, staffing, and achievement of improvement goals. The AVP will engage with other disciplines as necessary such as medical staff and nursing to remove barriers, align with the Case Management agenda, and promote desired outcomes. The AVP will be responsible for supervising and managing the Facility Support team. This role is responsible to determine the Support team focus to successfully execute the facility on-site support engagements. The AVP collaborates with others in Case Management to ensure the Facility Support team remains current and aligned with Case Management program and policy changes and new initiatives, tools, education, etc. The AVP is accountable for proactively identifying, documenting, and escalating (if needed) barriers to success. This person needs to possess strategic vision, data analysis skills, excellent communication skills, proven motivational capabilities, executive presence and a demonstrated track record of leading and motivating talent to delivering and attaining goals.
Strategically creates a Case Management facility engagement schedule based on data analysis from the HCA Case Management executive dashboard, obtains approval from Case Management leadership, and plans and implements the engagements. Designs, develops, and establishes a formal process for the Facility Support team approach, scope, role, and responsibilities in alignment and collaboration with Case Management leadership. Supports the identified facility and division Case Management teams to implement sustainable changes that result in improving the identified measure. Ensures data analysis, assessment, and root cause analysis to determine the opportunity, make appropriate recommendations on how to implement the change, and supports the facility in making the change. Provides leadership to theFacility Support teams focused on Case Management engagements. Develops and maintains engagement resource composition, schedules, and calendars. Ensures the engagement and accountability of the Division Case Management Director. Leads the Facility Support team to identify and implement sustainable changes that result in reduced process variation and improved outcomes. Develops and maintains effective working relationships with facility leadership to drive toward Case Management performance improvement objectives by establishing expectations, discussing strategic and sensitive issues, removing barriers and escalating issues. Develops and maintains effective working relationships with all pertinent Case Management leadership in each of the functional areas as well as Analytics, Education, Project Management, and Technology Serves as a subject matter expert and representative of HCA Case Management operational standards to ensure successful implementation and practice in a timely, efficient, and effective manner. Tracks, monitors and reports engagement status and outcomes. Leads and mentors a diverse workforce to enhance expertise, skills and measurable value. Works with Case Management Leadership to adjust Case Management strategies in response to changing business needs. Drives business decisions through the use of analytics. Develops a culture of high performance and continuous improvement that values learning and a commitment to quality. Communicates timely and effectively in a matrix environment. Escalates issues to Case Management leadership as necessary. Attracts, develops, and retains talent across the organization to ensure succession plan. Conducts onboarding and training of new team members.
· Bachelor of Science in Nursing is required; Master’s degree or equivalent experience preferred. RN with current state licensure Strong clinical background with 5 years of acute nursing experience required Minimum 5 years Case Management leadership experience required HCA Case Management Director experience preferred Experience with implementing and leading large-scale projects Superior interpersonal skills at the executive level and the ability to work within a matrix environment are a must Excellent communication and presentation skills - both written and verbal Nashville based , 80% travel
