The Case Management Director Facility Support is primarily responsible for the development and execution of a short term, facility based quality performance plan. The Director will provide consultation and education to the facility case management staff regarding process improvement opportunities related to foundational standards and government/regulatory compliance including the Joint Commission and Centers for Medicare and Medicaid Services (CMS). The Director will identify improvement opportunities while providing facility support to members of the case management team regarding case management standards and processes. The Director will also facilitate process standardization, optimizing technology efficiencies and reporting capabilities in a metric driven environment to promote improved patient outcomes, maximize resource utilization, and improve organizational efficiencies.
As an essential element to the overall success of the Case Management Program, the Director will consistently demonstrate a strong work ethic and commitment to excellence. This position will work to identify and support key strategies by leveraging data from various financial, clinical and operational systems. This position requires solid leadership, trust building, team building, as well as change management to influence and lead other HCA resources and business partners in order to ensure success of all organizational initiatives. This individual must excellent communication skills, proven facilitation skills, proven motivational capabilities, have an executive presence, and a demonstrated track record of delivering results and attaining goals. This position requires the ability to function with minimal supervision in a rapidly changing environment. The Director must be a Subject Matter Expert in case management. The Director must understand the clinical and financial aspects of case management and must be comfortable working with all levels of management. The Director will work directly with the facility case management directors in alignment with the Division and Corporate Case Management team.
This position requires an individual who is a self-starter and has the ability to manage multiple priorities, work with minimal supervision on projects and activities, demonstrates tact and diplomacy in situations of conflict and controversy and demonstrates excellent communication and presentation skills. This individual must be able to adapt quickly to change and be able to coordinate efforts across multiple stakeholders * Provides direct support to facility case management team related to case management processes and workflows to meet established goals * Acts as case management subject matter expert * Provides ongoing case management education related processes and workflow * Assist the director with evaluating and maintenance of evidenced based tools * Provide ongoing evaluation and assessment of facility practices, adherence to HCA Case Management standards and industry changes to ensure the enterprise remains current in processes * Assists the facility with action plan development and follow-up questions as requested * Offers suggestions and recommendations to resolve program challenges and obstaclesBSN with current state licensure Strong clinical background with 4-6 years acute care nursing experience required Relevant case management experience required Knowledge of audit and process improvement processes and practices In-depth knowledge of case management, discharge planning, care coordination, medical necessity criteria and utilization review practices and ability to incorporate successful current trends Knowledge and understanding of governmental, CMS, and JCAHO regulations Ability to communicate effectively in oral, written and electronic formats Ability to establish and maintain collaborative and effective working relationships with a focus on the importance of teamwork Excellent communication skills, both written and verbal Nashville base. travel required is 80%
