TheLead Clinical Social Workeris responsible for providing social work services to patients and their families as a member of a multidisciplinary team. In addition, the Lead Social Worker will serve as a supervisor for the activity therapy department and social work department. The Lead Social Worker will work collaboratively with other members of the hospital at problem solving potential complex behavioral health needs.
Social work interventions are focused on assessment, crisis stabilization, psycho-therapy, and discharge planning. At times, the social worker may provide service to different patient age groups, based on training and knowledge. In addition, the Lead Clinical Social Worker may provide supervision to other social workers, students, and interns. The Lead Clinical Social Worker will also carry case management duties on the behavioral health unit.
ESSENTIAL FUNCTIONS* * Participates in interviewing, orientation, training and mentoring staff. * Provides initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs and/or medical complex diagnosis. Ensures patient is progressing towards desired outcomes and identifies and resolves barriers that hinder effective patient care. * Works collaboratively with community agencies to provide services to assist with housing, financial, transportation, psychosocial, education, home health and DME needs of patients/families. * Engages and networks with community partners to increase communication, solicit feedback and strengthen relationships within the community * Develops and maintains database of community referral networks, outpatient partner providers and post-hospitalization treatment options. * Facilitates processes and procedures of department in order to strengthen optimal movement of patients throughout the continuum of care. * Identify process improvement opportunities, develops action plans and oversight of the implementation thereof in collaboration with the Manager. * Serves as a resource to all behavioral health staff in therapeutic process and the scope of clinical social worker practice. * Responsible for compliance with regulatory standards, TJC, CMS, DBHDS. * Develop and maintain tools to assess quality and compliance; identify improvement opportunities and implement action plans. * Reviews Integrated Assessments and conducts comprehensive psychosocial assessment focused on patient’s strengths, barriers, and supports which impact optimal functioning across spheres. Documents assessments, mental status examinations, and risks assessments in the medical record. * Coordinates and participates in treatment planning. * Monitors patient progress through the use of mental status examinations and risk assessments and coordinates discharge planning as evidenced by documentation. * Facilitates therapeutic interventions including process groups, individualized treatment interventions, and family meetings. Assesses patient progress and barriers to treatment as evidenced by documentation. * Has working knowledge of utilization management functions and performs these duties as assigned. * Provides for the safety of patients, visitors and staff and actively participates in quality management activities. * Reports to work as scheduled and on time.· * Assumes responsibility for completing all mandatory educational, health screening, CPR, and CPI requirements within designated time frame. * Demonstrates self-directed learning in order to nurture own professional developments, as well as meet the needs of the organization. * Other duties as assigned.
Master’s Degree from an accredited social work program or Master’s Degree in a related field that provided appropriate background and experience. Education should be clinically focused with study in human life span development and psychopathology.
· Current VA License as a Licensed Clinical Social Worker
· BLS for Healthcare Providers through the AHA
· Crisis Prevention Institute Certification is required within 30 days of hire
- At least the equivalent of 2 years of experience, which can include a graduate level practicum (20 hours per week for 2 semesters or 40 hours per week for 3 months).
- Experience must be focused in the area of inpatient hospitalization, partial hospitalization, crisis prevention, or outpatient practice.
