This role is responsible to identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN/LVN or MD. This is a case management position which includes the coordination of care and medical management consulting.

Primary Responsibilities:

  • Can contribute to the assessment of health status through gathering and recording of assessment data, observing, and monitoring and reporting of changes in condition to the supervising RN/LVN
  • Receive information from consumers about specific symptoms and questions
  • Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health literacy
  • Ask open ended questions, identify significant from insignificant responses, ask about other diagnoses, probe for additional information, and redirect as necessary
  • Uses existing procedures and facts to solve routine problems or conduct routine analyses
  • Depends on others for instruction, guidance, or direction
  • As Case managers, cases for patient follow up, Post Discharge Calls and Manage care
  • LVNs work hand in hand with Clinicians and Social workers for assistance in gathering information like authorizations, appointments, and complex cases
  • LVNs also triage referrals received. Plan on which Clinicians or Social workers to send and schedule if needed
  • Data collection from doctors for behavioral health and attaching POLST forms
  • Other duties as assigned

Required Qualifications:

  • Current unrestricted LVN licensure in CA
  • 2+ years of experience in clinical setting
  • Proficient with Microsoft Office (Excel, Word)
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation

Preferred Qualification:

  • 1+ years of experience with case management in either hospital, nursing home, hospice
