Role Purpose

The Care Coach, in collaboration with the interdisciplinary care team, is responsible for helping facilitate patient care along the continuum through effective transition planning. This position assists patients with achieving optimal health, access to care and appropriate utilization of resources to ensure patients receive the right care at the right time.

Responsibilities

Provides support to ambulatory care management programs; conducts patient outreach to individuals following a recent ED or inpatient stay; reviews patient discharge instructions and assesses understanding; completes medication review, schedules PCP follow up appointments and identifies and escalates clinical concerns.Identifies and manages patients' care needs per established protocols and department policies for Care Management Programs.Assesses and determines barriers to care; provides care coordination and assistance to patients to address concerns.Utilizes strong time management skills, prioritization skills, and critical thinking skills to perform patient outreach.
