Schedule

  • 7 on, 7 off
  • 9 am -7 pm (Monday – Friday), 7:00 am – 5:00 pm (Saturday and Sunday)

Minimum Requirements

  • Doctorate Degree in Pharmacy
  • PGY1 Pharmacy Practice Residency
  • RPh license / MTS

Preferred Qualifications

  • PGY2 Oncology Residency Training
  • BPS Board Certified Area of Specialty
  • 2-5 Years of experience

Role Purpose

Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.

Responsibilities

Assess the overall status of the patient?s health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.Participates in professional development and research activities.Contributes to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.Participates in the education and training of pharmacy staff, residents and students, and serves as a role model and mentor.Participates in the identification and implementation of targeted drug cost reduction programs.
