Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am – 5:00pm.
- Locate, outreach and engage members within the community that clinicians and care navigators have not been able to reach; create a positive experience and relationship with the member via telephonic and field-based interactions
- Assesses functional and social needs of members and collaborates to develop and implement solutions with the frequency established in the model of care
- Establish goals and a plan of care to meet identified functional and social needs
- Begin implementation of care plan
- Work collaboratively the multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care
- Works closely with clinicians to help bridge the gap for these previously un-engaged members and members with whom contact was lost
- Identify, refer, and coordinate available community resources to address non-medical needs and ensure patient access to services
- Completely and accurately document in patient’s electronic (, UnitedHealth Care) medical record
- Provide patients and family members with education regarding the need for follow up as appropriate during each patient visit
- Actively participate in organizational quality initiatives
- Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of care delivery (e.g. Pod calls)
- Demonstrate a commitment to the mission, core values and goals of UnitedHealthcare and its healthcare delivery including the ability to integrate values of compassion, integrity, performance

- High School Diploma / GED (or higher)
- Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
- Ability to travel up to 50% of the time for field-based work
- Abitlity to work Monday - Friday between 8:00am to 5:00pm (local)
- Must reside within 2 hours of Little Rock, AR
- 1+ years of knowledge of Medicare/Medicaid population
- Knowledge of culture and values of community
- Familiarity with the resources available in the community
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Ability to keep all company sensitive documents secure (if applicable)
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation.
- 3+ years of experience in working with at risk populations
- Must have resided within the state of Arkansas for 2+ years
- Field based experience
- Certified Nursing Assistant / Home Health Aide, or Medical Assistant
- Bilingual fluency in Spanish and English
- CHW Certification
- Experience working in managed care
- Ability to work with diverse care teams
- Exhibit independent clinical judgment, but the ability to consult and engage with other team members for improved patient outcomes
