The Community Health Improvement Team is partnering with libraries, recreations centers and other community organizations in St. Louis City and North County.

This role will focus on prevention education in areas such as substance use/misuse, safety and physical activity.

Role will require travel and ability to work some evenings and weekends.

Plans, develops and oversees the K-12 educational health prevention curriculum models implemented out of the department specifically focusing on fitness, nutrition, safety: intentional or unintentional injuries, tobacco use, substance abuse, and comprehensive sexuality to include STD's/HIV-AIDS.

Role Purpose

Responsible for the planning, developing and implementing of prevention community health education. Serves as a resource for school and community groups and an access point in delivering best practice programs.

Responsibilities

Plans, develops, coordinates, and oversees delivery of school and/or community health education and screenings. Consults with school community clients to determine health needs and assists in formulating and implementing plans to reduce risk factors.Partners with program staff, physicians, hospital administration, and relevant clinical disciplines and marketing teams to implement an integrated approach to managing priority populations.Establishes key relationships with community-based organizations focused on improving the health of their employees/school age children to gain efficiencies in providing programming and support to requesting agencies.Evaluates effectiveness of programs by implementing, monitoring and tracking program outcomes. Assesses health trends and ways to evaluate and focus on high risk and special needs populations.
