Shift: Varying - evenings, weekends, some day

Summary:

  • The Penn Medicine at Home (PMAH) Resource Home Health Aide (HHA) functions as an integral member of an interdisciplinary healthcare team that provides home health aide, skilled nursing, and rehabilitative care to clients, allowing them to receive the medical care required to remain at home. The PMAH Resource HHA will serve a hybrid population of patients who are medically complex and require extended hours of in-depth support and intermittent aide services for a short duration. The individual is responsible for performing home health or hospice visits in homes and facilities in a wide geographic area as necessary.
  • The Resource HHA is responsible for implementing a Plan of Care, assisting the patient/family with performing personal care tasks, performing light housekeeping (including help with laundry and meal preparation), and assisting with the activities of daily living. The Resource Home Health Aide is responsible for accurately documenting care activities and the timely submission of patient documentation.

Responsibilities:

Patient Care
  • Directly performs or assists with the patient's bathing, oral hygiene, and skin and hair care. Prepares light meals for patients and assists with eating. Assists with the patients' clothing, bedpan, commode, and urinal use. Responsible for emptying foley catheter drainage bag and recording the amount on the weekly Home Health Aide report when applicable.
Exercise and Transfers
  • Assists with home exercise programs to an assigned group of patients. Helps with the patient's ambulation and bed-to-chair transfer; turns patients in bed to positions of comfort.
Patient Environment
  • Performs household and other environmental services tasks specified in the Home Health Aide Plan that are essential to the entity. Assists with pre-poured medications, which are ordinarily self-administered. Provides respite care to relieve primary caregiver as assigned.
Documentation
  • Accurately documents care provided to patients on time according to Medicare/ TJC standards. Completes allotment of hours as ordered by the department and communicates schedule changes. Accurately documents and verifies all Home Health Aide Activity Sheets with signature and obtains caregiver or authorized facility signature. Oversees the timely submission of the 'Rap Check Report' to support government reimbursement.
Communication
  • Communicates any unusual findings and occurrences, apparent changes in patient's physical, mental, or social status, home safety hazards, refusal of care, or patient/family dissatisfaction to the primary nurse and Home Health Aide Supervisor. Consistently promotes accessible communication between the family, RN Case Manager, and Scheduling Coordinator.
Plan of Care
  • Follows the written care plan concerning days of the week and number of hours assigned for patient visits. Provides care only as outlined in the Plan of Care unless instructed by the Case Manager of the designee. Reviews biweekly IDT Plan of Care before providing care to patients.
Employee Travel
  • Employees must provide their own transportation to and from patients' homes and meet current state requirements for operating a motor vehicle.
Education
  • Attends annual in-services and completes a minimum of 12 hours of continuing education annually. Demonstrates understanding and adherence to PMAH procedures while meeting all competencies as defined for Home Health Aides.

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • One (1) year recent experience as a Nursing Assistant or Home Health Aide
  • OR Six (6) months experience as a Nursing Assistant or Home Health Aide commencing within two (2) months after completion of an approved Nurse Aide or Home Care Aide Training Program Graduate of a certified nursing assistant program-in lieu of certification must be a nursing student matriculated in an accredited BSN nursing program, having minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major, and continue to maintain a 3.0 GPA in all nursing major subjects.
