Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services. The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St. Louis Children’s Hospital.

The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff. Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along the continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient�s right to self-determination.
Preferred Qualifications
Role Purpose
Serves as a clinical resource facilitator to the Care Coordination Department with key area focus in: Level of Care/Medical Necessity, Observation Admissions/Hours, Outliers, Compliance Documentation Management Program. Works with departmental management to ensure designated aspects of the program are functioning in highest quality.
Responsibilities
Provides support to the case management program.Coordinates orientation for new employees.Conducts ongoing audits to ensure staff competency.Coordinates special projects.Minimum Requirements
