Registered Nurse Case Manager, RN, PRN
Join the Belleville and Shiloh Memorial Hospitals in providing Case Management services on site. The ideal candidate will have Case Management as well as Med/Surge or Medical Telemetry experience
- PRN position - Monday - Friday availability required - no weekends
- Generally 1 shift a week is available but looking for at least 1 shift every other week
Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including Children’s at Memorial for pediatric emergency care. It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial Hospital Shiloh, a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care.
Care Management includes Utilization Management (Utilization Review), Discharge Planning, Social Services, Clinical Outcomes (Care Managers), Discharge Transition Coaches, Nurse Practitioners, and Clinical Documentation Improvement.
Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
ResponsibilitiesAssesses, coordinates, and evaluates each patient's plan of care.Establishes, continuously evaluates, and documents an individualized discharge plan.Ensures clinical documentation and cost-effective use of hospital resources.
