Optum UT is currently seeking a Diabetic Education Program Manager (DPM) to oversee the Community Center Diabetes Education Program. Join this dedicated team of caregivers and providers and play an active role in improving overall health for the entire Medicare Advantage community.


Reporting to the Associate Director of Clinical Operations for the Wellness Team, the DPM will be leading an instrumental opportunity for members with Type I, Type II, and Pre-Diabetes. Supported by a small team of clinicians, and working closely with network providers, Medicare Advantage Members, and prospect members, the Diabetes Program Manager will find a supportive network of resources committed to improving community health and having a significant impact on the wellness of the entire Optum membership.


Primary Responsibilities:

  • The DPM will play an integral role in the operations of the Wellness Team in the following responsibilities:
    • Provide a patient-focused, educational benefit for the Community Centers, while serving as a consultant and resource for the entire Optum membership
    • Coordinate weekly classes at our Optum Community Centers as well as network provider’s offices. These classes require ensuring instructor availability, following the approved curriculum, monitoring goal progress of each of the individual members, and assisting with promoting participation throughout the network
    • Maintain and renew the current accreditation under the Association of Diabetes Care and Education Specialists
    • Promote positive interpersonal relationships with fellow teammates, providers, physicians and patients
    • Require daily travel in the greater Salt Lake, UT area to include but not limited to Sandy, Draper, West Valley, and Layton up to 50%

Required Qualifications:

  • Bachelor of Science in Nursing (BSN) from an accredited school of Nursing
  • Completed DSMES Program and Business Management Certificate within the last 2 years
  • Current and unrestricted nursing licensure in state of Utah
  • Current Certified Diabetic Educator through the Certification Board for Diabetes Care and Education (CBDCE) or the ability to obtain certification within one year of hire
  • Current CPR and/or BLS certification or ability to obtain certification within 30 days of hire
  • 2+ years of patient facing experience as a Registered Nurse
  • Access to reliable transportation
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:

  • Experience working with an elderly population
  • Experience with telehealth platforms and documenting in multiple EHRs
  • Knowledge of diabetes management: diabetes, pre-diabetes, congestive heart failure, hyperlipidemia, hypertension, GERD, gout, celiac, PCOS, chronic kidney disease
