Join BJC's new exciting and well supported initiative - "Evelyn's Hospice House at Home"
Do you want to make a difference in end of life care by providing supplemental support with Evelyn's House level of care? Allowing hospice patients with intensive symptoms to ability to remain at home? Patients receiving "Evelyn's House at Home" care require 10-12 hours of care per day and have uncontrolled symptoms that they are trying to manage at home.
The ideal candidate will have great assessment skills, IV skills and willing to grow into the position while building this well supported BJC initiative.
Position supports all BJC territories - must be or become dually LPN licensed in Missouri and Illinois.
Hires will initially work with the hospice team by being a support Float nurse for field, Evelyn's House and/or the palliative care education team.
Schedule
Weekend Option - Sat/Sun and 1 day during week 9a-9:30pm
Cell phone, lap top and safety support provided
IRS mileage reimbursement rates - currently $.655/mile

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose
Provides direct patient care activities including IV management, utilizing the nursing process, under the supervision of the RN.
Responsibilities
Implements treatments and procedures within the scope of practice for the Licensed Practical Nurse.Under the direction of the Registered Nurse, provides patient teaching and discharge planning. Participates in professional development and life long learning.Performs patient assessment and evaluation with regard to the patient?s age and developmental needs.Assists in developing and updating the plan of care with regard to the patient?s age and developmental needs.Implements plan of care with regard to the patient?s age and developmental needs.Minimum Requirements
