Summary:

  • Provides clinical navigation for identified patients within the Penn Medicine/LG Health system. Performs clinical and administrative functions to support the coordination of inpatient and/or outpatient care needs. Patient care includes care to patients in the following age groups: adult (19 through 64 years), and geriatric (65 years and above).

Responsibilities:

The Cancer System has established a new oncology care program in partnership with United Here Health (UHH). The foundation of the program is to ensure efficient and effective multidisciplinary care coordination for patients with cancer or with a suspicion of cancer. This care coordination will be primarily led through a newly budgeted nurse navigator who is dedicated to the Penn-UHH program. The nurse navigator position is budgeted and housed in the Cancer System Administration and reports directly to Megan Roy, Manager, Nurse Navigation. The position is primarily virtual but it may require 1-2 days per week on site at PCAM, PPMC or Penn Cherry Hill. The position will require onsite meetings at least once per month at the UHH Health Center in Atlantic City, NJ. The following is a list of specific responsibilities for this position. Given the patient population, Spanish speaking candidates are preferred. May be required to travel to Atlantic City, PCAM, PPCH as well as other parts of New Jersey once a month.

· Partners with the UHH Health Center, located in Atlantic City, NJ, the Cancer System New Patient Office, and multidisciplinary clinical departments to ensure that referred patients received efficient and effective access to Penn Medicine cancer providers in accordance to the goals of the Penn-UHH program. Such goals may includes but are not limited to the following.

  • Appointments are provided within 10 days of referral
  • Multidisciplinary appointment needs will be assessed for each patient with same day and same location coordination
  • Additional labs and testing will be coordinated with the New Patient office and clinical stakeholders
· Provides longitudinal nursing management support for patients on treatment at Penn and communicates effectively with the UHH Health Center team about the status of patient appointments and patient treatments.

· Communicates effectively with patients, care givers, UHH Health Promoters and the UHH Health Center team to ensure patient appointments are coordinated effectively and all access barriers are resolved prior to treatment. Utilizes SDOH to identify specific social determinants barriers and partners with stakeholders to address identified barriers and risks.

· Partners with patients, care givers, UHH Health Promoters, and the UHH Health Center team to provide effective education on disease process, treatment plans, symptom management, etc.

· Collaborates with the UHH and Penn Medicine clinical teams in weekly chart rounds of UHH patients on treatment at Penn Medicine

· Partners with the Cancer System leadership team to assess gaps and barriers of the program and to evaluate and implement solutions

· Partners with the Cancer System leadership team and UHH leadership team to ensure effective clinical communications and documentation through the Penn and Mt. Sinai EMRs and to ensure discrete data is collected effectively to ensure quality assurance and improvement is a focus of the collaborative Penn and UHH teams.

Credentials:

  • Registered Nurse - PA (Required)

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
  • And 3+ years experience as a clinical nurse Education Specialization: Nursing, Healthcare management or related field
  • Master of Arts or Science
  • And 5+ years experience as a clinical nurse Education Specialization: Nursing
  • Spanish speaking (preferred)
