The Associate Patient Care Coordinator is responsible for improving quality scores through the closing of gaps in care and result in better patient care through preventive interventions.

Schedule is Monday-Friday: 8am-5pm. Office located at 14100 Nacogdoches Rd, Ste 120 San Antonio, TX 78247

Primary Responsibilities:

  • Reviewing patient charts in electronic medical records to confirm gaps in care
  • Submitting records to close HEDIS gaps in care in online portals
  • Calling patients to inquire about gaps in care (preventive services) that patient may still need
  • Pulling, conditioning, and disseminating quality reports from Tableau
  • Collaborating with assigned clinics on HEDIS quality strategies
  • Reporting to direct supervisors scores and strategies. Sharing leading practices among team

Required Qualifications:

  • High School Diploma/GED or 5 years of customer service experience within general office
  • 1 year of experience with strong interpersonal and written communication skills
  • Basic experience with Outlook, Excel, Teams, and Word
  • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance.

Preferred Qualification:

  • Experience with electronic medical record system preferred. Especially eClinical Works (eCW)
  • Experience with Tableau or other reporting systems is a plus
  • HEDIS knowledge preferred
  • Good organizational skills
  • Bilingual (Spanish/English)
