As a Senior Patient Care Coordinator, you will be responsible for maintaining positive relationships with internal and external clients, being the liaison between Member and Practitioner, and ensuring that all documentations have been completed and filed in a timely manner.

This position is full-time (40 hours/week) Monday to Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 08:00am – 05:00pm. Our office is located at 655 New York Ave. Washington DC.

Primary Responsibilities:

  • Accurate, timely and organized maintenance of facility database
  • Completes the notification in the software program.
  • Ensures / verifies the member is eligible and qualifies for the skilled benefit
  • Benefit determination for all our facilities, covering the entire stat by ensuring appropriate utilization and consistent application of the benefits.
  • Benefit determination: involves receiving electronic assessment tool from the field Practitioners and ensures correct contract for each provider
  • Notifies Long Term Care Facility designated staff, providing the skill details both telephonically and electronically.
  • Ensures review / discharge dates are timely
  • Demonstrates understanding of relevant computer systems, applications, and platforms
  • Utilizes effective communication skills, demonstrating the ability to speak and write clearly and to listen actively
  • Demonstrates ability to identify and exceed customer expectations and improve the customer experience by responding in a timely and professional manner to all requests and concerns.
  • Consistently meeting commitments to customers, as well as escalating customer feedback when appropriate and building strong relationships.
  • Collaboration with case management and team as needed.

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 1+ years of healthcare industry experience
  • Experience using Microsoft Word and Microsoft Outlook in a professional environment (ability to create, edit, save and send documents and correspondence)
  • Experience in telephonic customer service
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Telecommuting Requirements:

  • Reside within a commutable distance to 655 New York Ave. Washington DC
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Ability to keep all company sensitive documents secure (if applicable)
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Preferred Qualifications:

  • 1+ years of administrative or clerical responsibilities in a professional environment
  • Familiarity with Microsoft SharePoint (ability to store, organize, share, and access information)
  • Previous experience with Medicaid/Medicare
  • Previous experience in electronic medical systems

Soft Skills:

  • Ability to utilize a Windows PC and learn new applications easily
  • Ability to work with changing priorities and workloads
  • Ability to take initiative and apply critical thinking skills
  • Ability to function independently and responsibly with minimal need for supervision
  • Excellent communication skills and appropriate phone etiquette
  • Ability to build and maintain relationships with clients, both internal and external
