Summary: TheMedical

Office Clerkfunctions as the Engineer for the Facilities Program and its

development within the real property and Master Planning (RP & MP) group.

Essential Duties

and Responsibilities:(Reasonable accommodations may be made to enable individuals with disabilities

  • Screen, record,

and redirect telephone calls using appropriate telephone etiquette.

  • Review all

incoming paperwork via patient inquiry, mail, and fax; add proper patient

identifiers to every page of paperwork; route paperwork to appropriate nursing

staff and providers; input notes into AHLTA and/or generate a telephone consult

into CHCS.

  • Scan results of

diagnostic tests and patient’s plan of care into electronic documentation of

patient’s notes in the patient’s electronic health record in AHLTA.

  • Schedule follow up

patient appointments and perform patient appointment notifications per the MTF

scheduling and notification procedures; coordinate patient appointments to

avoid scheduling conflicts.

  • Prompt

preventative exam appointments during routine patient check out if noted on

patient care plan.

  • Provide accurate

and timely technical and administrative reports and correspondence such as ER

records, outpatient studies/reports, and input of HEDIS (Health Effectiveness

Data and Information Set) information to providers for completion of patient

records.

  • Receive Health

Effectiveness Data and Information Set (HEDIS) results from the Tri-Service

Work Flow Form; input results into CHCS/AHLTA and generate a T-Con for the

Primary Care Manager (PCM) to sign off on the results.

  • Telephone outside

medical facilities requesting documentation for patients requiring follow up

care.

  • Telephone patient

to inform patient of medication refill placed by medical provider; schedule

follow up appointment due to lab and/or x-ray result and inform patient a

referral was approved by medical provider.

  • Reorganize

patient’s appointment schedule due to change in availability of medical

provider and telephone patient informing patient of the appointment change.

  • Review and monitor

Relay Health inquiries and requests from patients and forward request to

appropriate medical provider and nurse via Telephone Consult in AHLTA taking

approximately 10-12 minutes; set-up patient appointment request and follow up

with patient within 72 business hours.
