The Case Manager works with physicians and other members of the multidisciplinary team to develop a plan for each patient from admission to discharge. They lead the collaboration process which assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs through communications and available resources to promote quality, cost-effective outcomes.

Required Job Qualifications include: * Current licensure as a Registered Nurse. Compact license accepted * BSN preferred * CCM preferred * Minimum 2 years of clinical nursing experience * Previous case management experience required; acute care hospital case management experience highly preferred * Knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules preferred
