With 466 beds , we specialize in heart care at the Levinson Heart Hospital and emotional and mental health services through the Tucker Pavilion. Our ER is also a state-authorized Level II trauma center and we offer Central Virginia’s only Pediatric ER staffed 24/7 by pediatric-trained ER nurse and physician specialists.
The Joint Commission awarded Chippenham Hospital the Gold Seal of Approval and named us a Top Performer on Key Quality Measures for advanced primary stroke, heart attack, heart failure, pneumonia, perinatal care, hip and knee replacement and surgical care. We are also a Certified Chest Pain Center and a Level II Trauma Center , which means we provide fast, skilled care in life’s most difficult moments.
The Wound Healing Center at Chippenham Hospital has under its umbrella two departments, Outpatient and Inpatient. Outpatient is comprised of outpatient visits with oversight from physicians. We have two hyperbaric chambers. Within Inpatient we do consults generated by physicians and/or nurses, and we see patients with wounds, ostomy and incontinence. The Wound Healing Center team is made up of highly tenured clinical professionals, with more relevant experience (if totaled) than any other Wound Healing Center in the region.
The Case Manager/Registered Nurse in the Wound Healing Center : 1. Reviews health records for appropriateness of application of 2 Midnight Rule for initial review of all Medicare admissions. Performs continued stay reviews of trauma patients for appropriateness based on InterQual® criteria.
Performs admission and concurrent reviews for all payors and maintains legible documentation as directed by the Utilization Management Plan.
Reviews patient notes identifying and addressing any conflict in medical orders or lack of management plan.
Builds a rapport by providing continuity of care with patients and acting as their advocate throughout their hospital stay.
Collaborates between multiple care givers and fosters communication between medical teams and nursing staff.
Identifies and intervenes to coordinate care of trauma patients in relation to appropriateness of level of care, length of stay and resource utilization issues.
Identifies barriers to discharge and contacts relevant personnel to overcome these.
Initiates and monitors referral process of patients post discharge.
Identifies, tracks, and records Avoidable Days [AD], intervening to prevent or decrease as appropriate.
Initiates denial and appeal process in accordance with policy.

Diploma or Associates Degree from a professional nursing program (required)
BSN or current enrollment in a BSN program at an accredited school of Nursing (preferred) Licensure/Certifications:
Licensed or eligible for licensure in the State of Virginia as a Registered Nurse (required)
Case Management Certification through AACN (preferred) Experience :
5 years of recent clinical experience (required)
Commercial Utilization Management and/or Case Management experience (preferred) HCA Rewards:
