THE SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS OF THIS ROLE:
Provides initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs, mental health illness and or substance abuse needs.
Collaborates with the interdisciplinary treatment team and develops treatment goals and plans interventions that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, engagement in high risk social behaviors (i.e. substance abuse), limited finances and/or physical disabilities.
Educates patients and families to the patient’s needs post discharge and informs them of the alternatives available to meet those needs. Obtains consents, facilitates the discharge plan and completes the Discharge Information Form.
Provide individual/family sessions appropriate to patient’s needs.
Collaborates with community agencies, referral sources and continuing care providers to ensure continuity and coordination of care.
Completes clinical documentation in the patient’s medical record congruent with departmental policies.
Display professionalism in interactions with patients, families and staff.
Participates in clinical supervision as assigned by the Director of Clinical Services.
Maintains current knowledge of trends related to care coordination issues through attendance in continuing education and professional organizations.
Completes annual courses assigned in Healthstream in a timely manner.
May perform other duties as assigned.
- Valid Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) (required)
- Master’s Degree in Mental Health related degree from an accredited institution (required)
- Master’s Degree in Psychology, Counseling, or Social Work from an accredited institution (preferred)
- Certification to provide Supervision to other staff seeking licensure (required)
- American Heart Association Basic Cardiac Life Support (BCLS) Healthcare Provider Certification upon hire (preferred)
- TOVA/Crisis Prevention Institute (CPI) Certification upon hire (preferred)
- One year paid employment or a field placement in a health care setting (required)
- Two years or more paid employment or a field placement in a health care setting (preferred)
- Full- time (Monday-Friday; 8 hour shifts)
- Holiday Coverage (as required/needed)
