Position Summary:

The Nurse Case Manager utilizes their clinical skills telephonically.

Work in a positive office setting while still making a difference in member's lives. The Case Manager will collaborate and work closely with other healthcare professionals, including providers within the community and clinical team members internally. Additional Job Responsibilities:

The Nurse Case Manager is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members from our Federal PlansThe CM is responsible to evaluate the medical needs of the member in order to facilitate and promote the members overall wellnessThe CM develops a proactive course of action to address issues presented to enhance the member's short and long term outcomesBackground/Experience:

Minimum of 3 years clinical experience required Case Management experience preferred Education:

The minimum level of education required for candidates in this position is a Associate's degree or equivalent experienceLicenses and Certifications:

Registered Nurse (RN) with active state license in good standing requiredFunctional Experiences:
