Home Health Division: Penn Medicine Home Health care teams provide patients and their caregivers with the tools they need to perform rehabilitative procedures correctly, make appropriate lifestyle changes and provide proper care for their loved one. Our goal is to help patients recover as safely and quickly as possible, supporting independence as their conditions improve.

Penn Medicine Home Health services include:

Hospice Division: Our holistic philosophy takes a pro-active approach to pain and symptom management and recognizes that pain extends beyond the physical. We address the symptoms of each patient's illness as well as the emotional and spiritual concerns of the patient, caregiver and family — assisting patients and loved ones with the challenges that come with death and dying.

Hospice care provides a way for patients to focus on quality of life and take control of their health care decisions. Our multidisciplinary hospice team provides a wide range of care and support, allowing patients to live out their final months comfortably, in control, and with the dignity and respect they deserve.

Dual Infusion: Home care nurses play a crucial role in providing patients with the care they need in the comfort of their own homes however, a Penn Medicine dual infusion nurse takes it a step further by combining clinical expertise, compassion, and adaptability to provide high-quality care to patients receiving infusion therapy at home. Their dedication to patient care is truly admirable

Infusion: Penn Home Infusion Therapy's disease-specific clinical care teams are best in practice, allowing for expertise across oncology, nutrition, infectious disease, cardiology and pain management. This allows for the most specific and appropriate care possible. Some available therapies include:

·Hemophilia (factor therapy)

·Parenteral and enteral nutrition

·Intravenous gamma globulin (IVIG)

Position Description:

    • The Clinical Nurse 2 or Clinical Nurse-NCL is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model.
    • The Clinical Nurse 2 or Clinical Nurse-NCL possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum

    • The Clinical Nurse 2 or Clinical Nurse-NCL exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains.

    • The Clinical Nurse 2 or Clinical Nurse-NCL independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly.

    • The Clinical Nurse 2 or Clinical Nurse-NCL demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork
    • The Clinical Nurse 2 or Clinical Nurse-NCL works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients.
    • The Clinical Nurse 2 or Clinical Nurse-NCL seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies.


    Qualifications:

    Clinical Nurse-NCL (Non Clinical Ladder) track: requirements:

    • Diploma or Associate Degree in Nursing (ASN) from an accredited school of nursing

    • 15+ months of relevant professional nursing experience

    • Pennsylvania (or New Jersey) RN licensure (depending on hire location)

    • BLS/CPR as a healthcare provider, as per the American Heart Association

    • Have a car, valid driver’s license, and car insurance


    Clinical Nurse 2 track requirements:

    • Bachelor of Science in Nursing (BSN) from an accredited school of nursing

    • 15+ months of relevant professional nursing experience

    • Pennsylvania (or New Jersey) RN licensure

    • BLS/CPR as a healthcare provider, as per the American Heart Association

    • Have a car, valid driver’s license, and car insurance
