Summary:

  • The Program Assistant will provide support for multiple Community Health Worker (CHW) teams embedded within inpatient and outpatient settings. CHWs provide person-centered support to Penn Medicine patients to set and achieve health goals. Program Assistant duties include identifying, outreaching, and enrolling patients eligible to receive the IMPaCT CHW intervention. Duties will also include screening and enrolling participants in quality improvement (QI) initiatives, maintaining telephone follow-up with participants, data collection, survey administration, medical chart abstractions, data entry/cleaning. Additional tasks may include assisting with ongoing projects related to implementing IMPaCT in new settings. The Program Assistant may also support other center-wide projects and perform other duties as reasonably assigned.

Responsibilities:

  • Identifies, outreaches, and enrolls patients eligible for the IMPaCT intervention, ensuring that program enrollment goals are met weekly and annually
  • Uses the electronic health record to identify and produce reports of patients meeting eligibility criteria for CHW support
  • Supports patients in creating a health-related goal to work on with the CHW
  • Ensures accurate and timely documentation in specified platforms including REDCap
  • Provides support for IMPaCT quality improvement and evaluation initiatives, including:
    • Identifies, outreaches, and conducts assessment with participants
    • Completes data entry, data cleaning, and reporting

Credentials:

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • 1-2 years experience in health care, non- profit, or community-based organizations, and service in low-income communities
  • Bachelor of Arts or Science, Health Sciences, Social Work, Sociology, or Public Health
