Location: 150 Monument Rd - Bala Cynwyd

Hours: Per Departmental Needs - Mostly remote with occasional on-site or practice meetings.

Summary:

  • The Care Management Supervisor is responsible for managing a regional team of Clinical Care Managers that are dedicated to promoting organizational values of patient satisfaction by demonstrating courtesy and respect for families, visitors and other employees. The role includes hiring, onboarding and management of the Care Managers. The Supervisor collaborates with the PPiC Clinical and Administrative leads along with leaders from the hospitals, home care, and post-acute teams to assist in patient delivery. The Care Management Supervisor participates in coordinating and improving care management workflows and identifies opportunities to further develop or improve services.

Responsibilities:

Clinical Responsibilities:

  • Designs and implements care management support activities for high-risk patients with chronic diseases and high medical service utilization.
  • Collaborates with physician and practice staff in identifying appropriate patients for care management utilizing established criteria.
  • Initiates appropriate evaluation and management of emergency situations.
  • Creates, uses, and maintains patient-specific care management tracking system that identifies/ reports on patients who will benefit from care management services, the subsequent patient progress, and associated interventions.
  • Supports QA and maintenance efforts for patient registries and quality reporting. Works with the Clinical Analyst to summarize and analyze data from registries
  • Collaborates with all members of the health care team to resolve patient health, social, or behavioral health needs.

Supervisory Responsibilities:

  • Functions as a clinical resource for the Clinical Care Managers and the Social Workers by assisting the team in problem solving, identifying resources to support patient care, and collaborating with practice staff, Practice Managers and/or Providers when necessary to resolve patient issues.
  • Oversees the daily work demands of the team to prioritize assignments and assure patient outreach requirements are maintained (runs reports, distributes work assignments for coverage, etc)
  • Supports the department in developing and maintaining best practices by assisting with staff audits and ongoing education. Provides feedback to improve processes or clinical practice.
  • Manages team schedules/timecards, participates in the recruitment and selection process, completes introductory and annual performance reviews, and manages performance of team members on an ongoing basis.
  • Oversees and participates in the orientation and onboarding of new staff with regard to clinical experiences and education.
  • Supports and engages in frequent communication within the team and department, across Penn Medicine entities, and with local external agencies in order to facilitate seamless processes for patient care coordination within their region.
  • Reviews quality data and dashboards on a regular basis to drive improvement based on established goals and metrics.
  • Meets with Regional Leaders on a regular basis to provide updates on Penn Partners and Regional performance.
  • Works closely with Practice Managers to promote coordination and collaboration with the practice teams in delivering patient care.
  • Participates in relevant meetings with both internal and external partners as they pertain to care delivery.
  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
  • Other duties as assigned to support the unit, department, entity, and health system organization

Credentials:

  • Active PA or NJ RN license based on position location. Dual (NJ &PA) RN licensure required within 3 months of hire. RN (Required)

Education or Equivalent Experience:

  • Bachelor's Degree Nursing (BSN) And 5+ years of supervisory experience in a relevant health care setting. (Required)
  • Consider >8 years of relevant health care supervisory experience in lieu of Bachelor's degree.
  • 1+ years supervisory or management experience (Preferred)
